Humana, Interwell Expand Kidney Care Access for Medicare Members

This collaboration aims to provide Humana Medicare Advantage members with broader access to Interwell’s specialized services, focusing on the benefits of coordinated, patient-centered care. A key feature of this expanded agreement is enabling eligible members to access Interwell’s extensive network, which includes 2,000 nephrologists, renal care coordinators, and a multidisciplinary in-home virtual support team. This network is designed to offer specialized and ongoing support tailored to the needs of patients suffering from CKD and ESKD. By prioritizing value-based care, Humana aims to ensure that healthcare efforts are not merely reactive but are designed to prevent complications and improve the quality of life for patients. With a proven track record, Humana reports a 5% reduction in unnecessary hospital admissions in 2023, demonstrating the efficacy of such partnerships.

The Success of Value-Based Care
