The five-year project will focus on solutions to improve health outcomes and reduce costs. It will initially address chronic diseases and acute conditions that are prevalent in South Carolina.
In the initial programme, Medtronic and MUSC will work to standardise a comprehensive vascular disease care pathway in the continuum of patient care.
This is expected to result in a model that will enable the earlier identification and treatment of patients with vascular disease. The model will also be designed for follow-up care.
